COVID Vaccination Clinics

NPS 12+ Clinics

In an attempt to increase access and equity a mobile vaccine clinic initiative has been established in partnership with the Norwalk Community Health Center. NCHC will provide mobile Covid vaccine clinic services to eligible students with parental consent. The clinics will be held at Norwalk middle and high schools during the school day. After receiving the vaccine, the students will be observed by a nurse for 15-30 minutes before returning to class.

Please find consent forms below for students 12 + to receive their Pfizer Covid vaccine via the Norwalk Community Health Center mobile covid vaccine clinic.

NPS 5-11 Clinics

NPS is excited to partner with Griffin Health to offer children ages 5-11 the pediatric Pfizer Covid vaccine authorized by the CDC and FDA. Vaccines will be provided to eligible students with parental consent.

Please bring a completed consent form and prevaccination checklist to the clinic. After receiving the vaccine, students will be observed by a nurse for 15-30 minutes. Limited quantities are available. No appointment necessary. No cost.

Please be reminded that students who are fully vaccinated (it has been at least 2 weeks since their final vaccine) are not required to quarantine after exposure to a Covid-positive individual provided they remain symptom-free.
